Photo Release

PHOTO RELEASE 1. As a benefit for members and guests of Grace Chapel, Inc. (“Grace”), Grace is pleased to provide a photographer who will be taking individual and family photographs. 2. In consideration for my being permitted by Grace to participate I/we individually and on behalf of any of my/our minor children agree to release and hold harmless Grace, its respective agents, officers, directors and employees (the “Releasees”) from any and all claims I/we might have as a result of my participation including the release or dissemination of the photographs, mine/our and my/our children’s likenesses or any portions thereof. 3. By signing this Photo Release, I/we represent that I am/we are at least 18 years of age. If I am/we are signing this Release of Liability on behalf of a minor or child under the age of 18, I/we represent that I/we are the parent or legal guardian of such minor. I HAVE CAREFULLY READ THE FOREGOING PHOTO RELEASE, UNDERSTAND ITS CONTENTS AND SIGN IT WITH FULL KNOWLEDGE OF ITS SIGNIFICANCE.
